Job description

Assisted Home Health and Hospice is seeking a full-time CHHA (Certified Home Health Aide) to care for patients at the Allara Senior Living Facility in Rancho Cucamonga.

Applicants must possess a current Home Health Aide Certificate issued by the California Department of Public Health.

This position is full-time, working 40 hours per week.

Shift: Monday – Friday, 8:00 am – 4:30 pm, with a 30-minute lunch break.

Compensation: $20 per hour.

The impact you will make:

  • Assist patients with personal hygiene, elimination, nutritional support, ambulation, and exercise according to individual needs and care plans.
  • Ensure the delivery of safe and quality CHHA services as per the written plan of care.
  • Communicate regularly with RN, Rehab Case Manager, or Clinical Supervisor regarding changes in patient condition or needs.

Who you are:

  • Completion of an approved training program in accordance with Federal/State law.
  • Current, active California CHHA certification, including a criminal background check.
  • Current CPR certification, health clearance, and completed skills assessment.
