SYNERGY HomeCare is seeking a Certified Home Health Aide or CNA (Active-FL Licensed) for weekend shifts in Melbourne, Florida. You will deliver compassionate, one-on-one in-home care, helping patients with daily living activities and emotional support, while collaborating with the clinical team to ensure quality care.

The ideal candidate holds an active Florida CHHA or CNA license, has at least 1 year of related experience, and can lift up to 50 pounds.

About the Role

As a Certified Home Health Aide or CNA (Active-FL Licensed), you will play a vital role in our team, providing one‑on‑one care to our patients in their homes. Your duties will include assisting with daily living activities, bathing, dressing, grooming, and providing emotional support to our patients. Responsibilities

• Provide personalized care to patients in their homes, focusing on their unique needs and preferences

• Assist with daily living activities, such as bathing, dressing, and grooming

• Monitor and report any changes in patients’ conditions to our clinical team

• Develop and maintain positive relationships with patients, families, and healthcare professionals

• Maintain accurate records of care provided and patient progress

• Collaborate with our interdisciplinary team to develop and implement patient care plans

Requirements

• Active FL licensure as a Certified Home Health Aide or CNA

• High school diploma or equivalent required; degree in healthcare-related field preferred

• Minimum 1 year of experience as a Certified Home Health Aide or CNA; experience in home healthcare setting a plus

• Ability to lift up to 50 pounds and perform essential job functions with reasonable accommodations
