Job summary

Anchor Health is seeking a Certified Home Health Aide with Hospice experience to join our team servicing the Livermore, Pleasanton, Dublin, San Ramon area.

You will provide compassionate in-home care, assist with bathing, hygiene, transfers, meals, and daily activities, and complete documentation. Travel to patients’ homes is required; bilingual abilities are a plus.

Qualifications

• High School diploma or equivalent; CHHA program completed.
• 1–2 years direct patient care experience.
• BLS certification required; able to communicate effectively.
• Bi-lingual is a plus.
• Valid California driver’s license with insurance; travel required.

Responsibilities

• Completes charting and timely documentation.
• Assists patients with bathing, hygiene, and personal care.
• Assists with transferring and ambulation.
• Assists with prescribed exercises as taught by health professionals.
• Prepares meals and assists with eating.
• Assists with toileting and related needs.
• Prepares household tasks to support self-care at home.
• Reports changes to Case Manager or Team Manager.

We are currently seeking a Certified Home Health Aide with Hospice experience to join our San Diego team!

Responsibilities:

• Completes charting and completion of appropriate documentation timely.
• Assists patients with bath, mouth care, skin, hair and all aspects of personal hygiene.
• Assists patients with transferring and ambulation as needed.
• Assists with prescribed exercises which patients and CHHA have been educated in by appropriate health professionals.
• Prepares meals and assists patients with eating as required.
• Assists patients in toileting including use of bedpans, urinal and commodes.
• Prepares household services that will facilitate the patient's self-care at home and are necessary to prevent or postpone institutionalization.
• Reports changes in patient condition and needs to the Case Manager or Team Manager.

Qualifications:

• High School diploma or equivalent. Completion of CHHA program.
• 1 to 2 years direct patient care experience in an institution or home care setting.
• Appropriate understanding of patient care requirements within certification.
• BLS required. Ability to communicate effectively verbally and in writing.
• Able to manage time effectively.
• Bi-Lingual is a plus.
• Current California driver's license and proof of insurance.
• Travel required.
