Assisted is seeking a per diem Certified Home Health Aide (HHA) for our Covina branch to conduct approximately 10-15 visits per week.

You must possess a Home Health Aide Certification through the California Department of Public Health.

The Impact You Will Make:

• Assist patients with personal hygiene, elimination, nutritional support, ambulation, and exercise based on their needs and care plans.
• Ensure the safe and quality delivery of services as per the written plan of care.
• Communicate regularly with RN, Rehab Case Manager, or Clinical Supervisor regarding patient condition or needs.

Who You Are:
