Job summary

Trilogy Health Services in Boonville, IN is seeking a Certified Assisted Living Associate (HHA) to provide compassionate, direct care to Assisted Living residents, support daily living activities, monitor health, and assist with medications as directed by care plans.

You will collaborate with a multidisciplinary team to maintain a safe, dignified, and positive environment, while upholding quality standards and resident comfort in a memory-care focused setting.

Qualifications

• High School Diploma or GED/HSE preferred.
• Indiana Home Health Aide Certification (HHA) is required.
• 0-1 Years of relevant experience preferred.

Responsibilities

• Assists AL residents with daily living activities, including bathing, dressing, grooming, toileting, and mobility; provides medication reminders and follows care plans.
• Monitors and documents vital signs (blood pressure, heart rate, temperature) as directed by nursing staff.
• Provides companionship and emotional support to residents, promoting a positive and nurturing environment.
• Assists residents during mealtimes, serving food and ensuring dietary needs are met.
• Helps with mobility and transfers using proper body mechanics and safety protocols.
• Maintains a clean, safe, and organized environment in residents' rooms and common areas.
• Follows facility policies and safety guidelines to provide high-quality care.

POSITION OVERVIEW

The Certified Assisted Living Associate (HHA) provides care and support to Assisted Living and Assisted Living Memory Care residents, ensuring their comfort, dignity, and safety. The role includes direct patient care, emotional support, and collaboration with healthcare professionals to maintain a positive environment for residents.

KEY RESPONSIBILITIES

• Assists AL residents with daily living activities, including bathing, dressing, grooming, toileting, and mobility. Additionally, assists with medication reminders and ensures AL residents follow prescribed healthcare plans.
• Monitors and documents vital signs (e.g., blood pressure, heart rate, temperature) as directed by supervising nursing staff.
• Provides companionship and emotional support to AL residents, promoting a positive and nurturing environment.
• Assists AL residents during mealtimes, including serving food, feeding if necessary, and ensuring dietary needs are met.
• Helps with mobility and transfers, using proper body mechanics and safety protocols.
• Maintains a clean, safe, and organized environment in AL residents' rooms and common areas.
• Complies with facility policies, state regulations, and safety guidelines to provide the highest standard of care.
