Certified Home Health Aide For Home Health Visits

Physicians Choice Home Health is seeking a Certified Home Health Aide to join our dedicated home healthcare team. In this role, you will provide quality home health visits to patients in their homes, working under the supervision of a registered nurse or therapist. You will observe patients, report and document observations and care provided, ensuring safe, continuous, and high-quality care.

As a paraprofessional member of the home care team, responsibilities include personal care services such as bathing, grooming, oral hygiene, assisting with mobility, and supporting medication management, among others. The role requires adherence to documentation standards and safety protocols.

Requirements:

• Completion of a state-approved training and competency evaluation program or equivalent
• Proof of registration on the Home Care Aide Registry (preferred)
• At least 18 years old
• Ability to read, follow written instructions, and document care
• Self-directed with minimal supervision
• Empathy and compassion for patients' needs
• Current CPR certification
• Ability to travel to patients' homes
