Responsibilities

• Perform light housekeeping, meal preparation, and medication reminders per care plan.
• Follow the individualized care plan provided by the nurse.
• Report changes in the client’s condition to the agency and document observations regularly.
• Maintain accurate and timely documentation of visits and services.
