Job summary

Ascend Hospice & Palliative Care NJ is hiring a Certified Home Health Aide (CHHA) to support patients in home and cared environments. You will assist with ADLs, meals, hygiene and mobility under nursing supervision.

The position requires a State Certified CHHA license, reliable transportation, and willingness to travel. Hospice/home health experience is preferred but not required, with strong compassion and communication essential.

Qualifications

• Must have a State Certified CHHA license.
• Experience in hospice or home health is preferred.
• High school diploma or equivalent required.
• Must be willing to travel and have reliable transportation.

Responsibilities

• Provide or assist with ADL activities: personal hygiene, grooming, mouth, skin care, nails.
• Assist with toileting, positioning and transfer activities; turn and reposition bed-bound patients.
• Aid with prescribed exercise and ROM as taught by Hospice Nurse.
• Support light housekeeping in patient unit and laundry as needed.
• Assist with meals and feeding; monitor intake.
• Measure temperature, pulse and respirations under supervision.
• Report changes in patient condition to Hospice Nurse and document appropriately.
• Assist or remind patient to take medications in line with regulatory guidance.

Now Hiring - Certified Home Health Aide (CHHA) -

This Certified Home Health Aide will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.

• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.

• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.

• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.

• Feeds and assists with meals.

• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.

• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.

• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.
