Ascend Hospice & Palliative Care NJ is hiring a Certified Home Health Aide (CHHA) in Ocean County. The role involves direct patient care, ADL support, mobility assistance and light housekeeping within the patient’s home or facility under Hospice supervision.

Qualified candidates must hold a State Certified CHHA license and are expected to have experience in hospice or home health; high school diploma or equivalent required. Travel may be required, with competitive compensation and benefits.

Qualifications

• Must have State Certified CHHA license.
• Experience in hospice or home health is preferred.
• High School Diploma or equivalent required.

Responsibilities

• Provides or assists in ADL activities including personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting, positioning and transfer activities and bed-bound care.
• Assists with prescribed exercises and ROM as taught by Hospice Nurse.
• Light housekeeping of patient unit including changing bed linens and laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports changes in patient condition to Hospice Nurse and documents appropriately.
• Assists or reminds patient to take medications in compliance with regulations.

Now Hiring - Certified Home Health Aide (CHHA) - Ocean County

This Certified Home Health Aide will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.
