Job summary

Affinity Hospice Care Inc. is seeking a Certified Home Health Aide to provide compassionate, hands-on care in patients' homes under the supervision of a Registered Nurse.

You will assist with daily personal care, safety, and comfort, supporting quality of life and easing the family's burden. The role is per diem with a visit-based rate of $35-$45, offering a flexible schedule, paid time off options, and potential transition to full-time based on performance.

Qualifications

• Californian Certified Home Health Aide certificate is required.
• Current CPR certification with AHA.
• Valid driver’s license with a reliable vehicle.
• Age 18 or older.
• Ability to read and follow written instructions and document care accurately.
• Comfortable working independently with minimal supervision.
• Genuine empathy and patience for ill or frail patients.
• Professional and attentive to personal hygiene standards.

Responsibilities

• Provide personal care including bathing, grooming, oral hygiene, dressing, and skin care.
• Assist with mobility, exercises, and range-of-motion activities per the care plan.
• Support meals, from preparation to feeding assistance.
• Take and record vital signs as directed.
• Help patients with medication self-administration.
• Keep the living space clean, safe, and orderly.
• Document care provided per organizational standards.
• Perform other duties as required for patient services.

Requirements
