Description Minimum Qualifications:

1. High School diploma or equivalent required.
2. Successful completion of a home health aide training program meeting national standards required.
3. Home Health Aide certification in the State of New Jersey required.
4. BLS certification required within 30 days from the date of hire or transfer.
5. Valid NJ driver's license, registration, and insurance required.
6. Safe driving record also required.
7. Minimum 1 year of experience working with the frail or elderly.

Position Summary

The PACE LIFE Program Home Health Aide provides assistance with activities of daily living, restorative, and supportive care to preserve the home environment. This role reports the health status of participants and ensures a safe environment. The Home Health Aide must follow the care plan set by the primary nurse and adhere to established standards. The position emphasizes quality customer service, participation in performance improvement, teamwork, and cooperation.

Qualifications

• Education: High School diploma or equivalent; completion of a nationally recognized home health aide training program.
• License/Certification: Home Health Aide certification in New Jersey; BLS certification within 30 days of employment; valid NJ driver’s license, registration, and insurance; safe driving record.
• Experience: At least 1 year working with the frail or elderly. Proficiency in clinical applications is preferred; training will be provided, and skill development is expected within 30-60 days of hire.

Performance Expectations

Demonstrate technical competencies as per assessment and evaluation tools.

Work Environment

This role requires effective teamwork within an interdisciplinary model, collaborating with clinical and social service disciplines. The work setting is an Adult Day Health Center and primary care clinic with moderate noise and controlled temperatures. The position requires normal vision and hearing, effective communication, manual dexterity, eye-hand coordination, and the ability to lift/carry up to 30 pounds using proper body mechanics.

Reports to department leadership.
