Job summary

Premier Health Partners is seeking a Home Care Aide (Certified) to support patients in Miami/Clark County and Central Montgomery County. You will work under a licensed nurse to perform basic nursing tasks and be assigned cases by scheduling or a supervisor.

The role requires high school completion or GED, plus CNA/STNA and CPR certification. OBRA 87 compliant training, reliable transportation, and the ability to lift up to 50 lbs are expected.

Qualifications

• Education: High School completion / GED required.
• Preferred: Home Care Aide Certificate.
• Must have completed the ninth grade and be able to read, write, and follow written instructions and document care given.
• Passport or ESP or Elderly Services Program staff: Must have obtained a high school diploma or completed GED equivalent.
• Licensure / Certification / Registration: CNA or STNA; CPR Certification.
• Must be self–directed and capable of working with little direct supervision.
• Must have good physical and emotional health, able to stoop, bend, lift up to 50 lbs, and maintain good personal hygiene.
• Must demonstrate a sincere interest and sympathetic attitude toward caring for the sick and disabled at home.
• Must meet all agency personnel policies and have reliable transportation.

Experience

• 1–3 years of job–related experience.
• Completed a basic aide training program that meets OBRA 87 requirements.
• Must possess a Certificate of Course Completion or have one year of hospital or skilled nursing home experience.
• Medicare, Medicaid, and Waiver aides must complete a two–part competency evaluation; score at least 70% on the written portion.

Knowledge & Skills

• Must complete the written portion of the competency evaluation with a score of 70% or greater.
• Agency must complete competencies.
