• Some high school
• Certified Home Health Aide (HHA) or Personal Care Aide (PCA)
• Current Photo ID & Social Security card
• Drug screen (last 6 months)
• Full physical (last 6 months) 2 PPD's, Rubella & Rubeola titers
• Two references
