Systole Health is transforming women’s heart health with group telehealth. We offer longer, virtual appointments which include health coaching, medication management and community support to improve blood pressure, cholesterol, blood sugar and weight. Working as a duo, the provider and health coach facilitate group visits through structured programming and goal setting.

We seek a full-time health coach to join the clinical team and drive health coaching with our patients. In close partnership with our team, you will help us design, test and improve our core patient programming and coaching to empower patients to make heart healthy lifestyle changes.

What You’ll Do

• Coach patients on healthy behavior change, including SMART goal setting, heart healthy habit formation, medication adherence, and remote monitoring in shared medical appointments.
• Facilitate group discussion and peer support during each appointment.
• Drive preparation and follow-up from appointments, managing the asynchronous communication with patients and providers in a timely manner.
• Clearly documenting appointment notes, patient updates, escalations and treatment changes within electronic medical records.
• Manage and facilitate engagement within community forums.
• Collaborate with the team to deliver a patient-centered experience and retain 90% of patients within the coaching program.
• Accurately maintain patient progress tracking in team systems.

We’re looking for an individual with..

• Certified Health Coach (e.g., ACE, NASM, Wellcoaches) or equivalent certification. Minimum of 3 years of experience coaching clients.
• Previous experience in managing a group (ie: previous facilitation experience, yoga teacher, instructor, etc.)
• Ability to independently manage a weekly schedule, including group sessions, 1:1 follow-ups, prep time, and asynchronous communication with patients and team.
• Clear and concise communication skills, with experience with motivational interviewing.
• Strong problem solving skills with an interest in continuous process improvement.
• Ownership mentality, driving assigned tasks and loops in relevant team members as needed to move initiatives forward — without dropping details.
• Passion for women’s health or heart health.
• Remote contract to hire position, must be located within the US.
• Compensation: $28 per hour.
