Responsibilities

The Certified Case Counselor II - MCM will provide direct case management services to patients at the Peter Ho Memorial Center. Responsibilities include offering client-centered services, linking clients with healthcare, psychosocial, and other services provided by trained professionals, including both medically credentialed and other healthcare staff. The role supports the clinical team by providing resources, information, and performing various steps of medical case management such as intake completion, psychosocial assessments, developing initial individualized client service plans, conducting client advocacy and retention activities, referral follow-ups, re-evaluation of patient needs/discharge, treatment adherence counseling, and proper case disposition. The counselor maintains documentation in systems like EPIC, NJDOH (CareWare), and Ryan White Grant reporting systems (CAHMPS), ensuring accurate and prompt data entry. The role also involves educating and consulting with service providers and consumers about HIV disease and meeting consumers’ needs, including treatment adherence counseling for complex HIV/AIDS treatments. Client-specific advocacy and review of service use, via face-to-face, telephone, and other communication methods, are also included.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

  1. Minimum of two years’ experience in case management, discharge planning, or related health fields.
  2. Master’s degree required; Master’s in social work or other health-related field / LSW preferred.
  3. Ryan White Case Management Certificate obtained within 6 months of hire preferred.
  4. Basic to intermediate computer skills (Microsoft Office: Word, Excel, PowerPoint, email, etc.) required. MCM training within 3 months of hire and maintained current.
