Position: Centralized Order Entry Pharmacist (Work From Home)

Schedule: Overnight, 2030-0700 EST, Wednesday to Tuesday (7 on/7 off)

Job Responsibilities
  • Monitor, evaluate, and make recommendations to ensure appropriate and effective pharmaceutical use for patient care.
  • Use the medication process to assess, plan, intervene, implement, and evaluate patient care in compliance with hospital policies and protocols.
  • Provide patient care activities to ensure safe and effective drug therapy.
  • Enter orders accurately and timely into the computer.
  • Follow procedures for non-formulary, target, restricted, and investigational drugs.
  • Screen for drug interactions, allergies, order duplications, appropriate diagnoses, and renal/liver function prior to entry.
  • Address facility queues promptly.
  • Investigate and report adverse drug events and medication incidents.
  • Review and interpret physician orders using patient profiles.
  • Monitor intravenous drug compatibilities, concentrations, and rates.
  • Document interventions and follow-up actions.
  • Assess orders for age-specific appropriateness from neonatal to geriatric patients.
Qualifications
  • Bachelor’s degree in pharmacy from an accredited college required.
  • PharmD preferred.
  • One year of hospital experience preferred.
  • Meditech experience preferred.
  • State Board of Pharmacy license required (Florida license necessary).
