Description

Hello! Thank you for your interest in joining the Wellspring team as a Centralized MDS Coordinator. As a statewide nonprofit serving seniors, families, and children in need of support, we take a whole-person approach to our organization and delivery of our services — because we believe we must find the strengths in people and build on these for lasting change to occur.

As an MDS Coordinator with the Wellspring Skilled Nursing team, you’ll be part of a team of nurses and senior support staff committed to helping seniors and families within the state of Michigan. In this position, you'll be directly supporting your community to provide services to seniors. You will need a nursing degree from an accredited college or university, Registered Nurse (RN) or Licensed Practical Nurse (LPN) license, and a minimum of 2 years of experience in a skilled nursing or long-term care setting. MDS experience is preferred.

Responsibilities of a Centralized MDS Coordinator include:
  • Conducting and coordinating MDS schedule and assessments for multiple campuses
  • Ensuring completion of MDS based on RAI guidelines, including dating and signing each MDS to indicate completion (LPN completed MDSs will be co-signed by RN)
  • Ensuring that quality MDS information is transmitted based on RAI guidelines
  • Reviewing and completing comprehensive care plans
  • Communicating daily with campus clinical teams to complete significant change assessments
  • Overseeing that all SNF care plans are established according to facility and regulatory guidelines and are reviewed based on residents' MDS, reflecting their current condition
  • Participating in facility teams to complete monthly triple checks
  • Being cross-trained in the centralized admissions process and participating in the admission weekend on-call rotation
