Responsibilities

• Referral Management: Receive, review, and process referrals; monitor referral activity; maintain referral tracking systems; ensure timely response to referral opportunities.
• Intake Coordination: Coordinate all aspects of intake from referral through admission; review clinical, financial, and insurance information; verify referral documentation requirements.
• Clinical and Placement Review: Review referrals for level of care; collaborate with facility clinical teams; support continuity of care.
• Case Management & Census Support: Support transition-of-care initiatives; communicate with families; support readmission reduction; assist census goals.
• Managed Care & Insurance Coordination: Verify payer source; coordinate authorization requirements and approvals.
• Customer Service & Relationship Management: Provide exceptional service; maintain relationships with hospitals and community partners.
• Reporting & Compliance: Maintain referral tracking reports; monitor conversion rates; ensure HIPAA compliance.
