Responsibilities

• Referral Management: Receive, review, and process referrals; monitor activity and maintain tracking systems.
• Intake Coordination: Coordinate intake from referral through admission; review insurance and financial details.
• Clinical and Placement Review: Assess referrals for appropriate level of care and admission suitability.
• Case Management Support: Support transition-of-care initiatives and communicate with families.
• Census Development: Assist facilities in achieving census goals and monitor admission activity.
• Managed Care & Insurance: Verify payer sources and coordinate authorization requirements.
