Oak Street Health is seeking a Center Medical Director in Waterbury, CT to lead the clinical team, practice primary care with 80% clinical time and 20% administration, and drive high-quality, evidence-based care. The CMD will mentor providers, lead center-level initiatives, and partner with regional leadership to meet strategic objectives.

You will manage daily clinical workflows, support care planning and metrics, and foster a positive, growth-oriented center culture while ensuring

Qualifications

• MD, DO, NP or PA graduate.
• Board certification in Internal Medicine or Family Medicine for MD/DO; Family/Adult/Gerontology for APN.
• Experience in clinical leadership roles.
• Experience using a metrics-driven approach to patient care and quality projects.
• Commitment to older adults and to low-income patients.
• US work authorization.
