Job summary

Oak Street Health seeks a Center Medical Director to lead the clinical team at our center, delivering high-quality patient care and fostering a great place to work. The CMD will balance primary care with administrative leadership, spending about 80% with providers and 20% on operations.

You will mentor clinicians, drive evidence-based care, and partner with regional leadership to execute strategic initiatives.

Qualifications

• M.D., D.O., or APN/PA graduate with board certification in internal medicine or family medicine.
• Experience in clinical leadership roles and coaching providers.
• Experience using metrics-driven approaches to improve quality of care.
• Commitment to older adults and low-income patient populations.

Responsibilities

• Lead the clinical team at the center and mentor providers.
• Drive adherence to Oak Street’s care model and evidence-based guidelines.
• Partner with regional leadership to execute strategic initiatives and improve patient outcomes.
