Summary

The Behavioral Health Case Manager provides comprehensive support and coordination of care for clients facing mental health challenges. This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure the delivery of effective and client-centered services. The Case Manager is dedicated to facilitating clients' recovery and promoting their overall well-being and independence.

Duties And Responsibilities
  • Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
  • Manage case assignments, draft service plans, and review case progress regularly.
  • Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
  • Collaborate with therapists, clients, and family members to meet behavioral health needs.
  • Facilitate clients in achieving personal independence and developing daily living skills.
  • Perform other duties as assigned.
  • Uphold documentation and billing practices as per CARF, CMS, and training guidelines.
Supervision

This position does not supervise staff. The Behavioral Health Case Manager reports to assigned Operations Managers.

Education
  • High school diploma or equivalent required.
  • A college degree in Human Services is preferred.
Certification
  • CPR certification is preferred.
  • NPI number/Ohio Medicaid ID is required (instructions to apply will be provided).
Experience
  • Minimum of 3 years in behavioral health or related field preferred, but not mandatory.
Background
  • Must pass all Federal and State background checks according to company policy.

  • Flexible scheduling options to accommodate personal availability, allowing for as little or as much work as desired.
