About the Position:

The CCSP Care Manager Nurse assesses and determines appropriate community-based services for Medicaid recipients or potential Medical Assistance Only clients within the Elderly and Disabled Waiver Program. The role involves working with an interdisciplinary team across large geographic areas, potentially including extensive travel. Supervisory responsibilities may be assigned by the Program Director.

Major Job Duties and Responsibilities:

1. Receive referrals for initial intake of CCSP services.
2. Schedule and conduct face-to-face interviews with clients at their residence, hospital, long-term care facility, or other appropriate locations.
3. Perform comprehensive assessments covering social, functional, physical, mental, nutritional, and environmental factors, considering client preferences.
4. Review Medicaid eligibility and determine cost sharing based on established guidelines.
5. Explain program details to clients and obtain necessary signatures.
6. Analyze medical and social information, consulting with professionals as needed.
7. Participate in interdisciplinary team meetings to discuss client needs.
8. Determine suitable services and settings to support clients' health and functional status, referring to additional resources as appropriate.
9. Develop initial care plans in collaboration with Case Management.
10. Conduct reassessments following guidelines.
11. Coordinate with Provider RN and other healthcare providers for disease management and referrals.

Minimum Job Requirements:

Education & Experience: Completion of nursing education with current LPN/RN license in Georgia. At least two years of experience in geriatric nursing, community health, long-term care, or chronic diseases in adults.

Certification: CPR certification required.

Knowledge and Training: Ability to analyze data, identify problems, and communicate insights effectively. Knowledge of CCSP/SOURCE programs is preferred.

Computer Skills: Proficiency in basic Windows applications and navigation.

Transportation: Reliable transportation is required.

Job Type: Full-time

Pay Range: $53,000 - $71,900 per year
