Job summary

Northeast Kingdom Human Services Inc in Vermont seeks a Veterans Outreach Case Manager, a part-time role serving veterans and active‑duty personnel with travel between Derby and St. Johnsbury. You will provide direct case management, home visits, outreach, and help connect clients to services.

The VOCM will coordinate with VA centers and community providers to ensure integrated care for behavioral health and substance use, assigning a Principal Behavioral Health Provider to every veteran served.

Qualifications

• Bachelor’s degree or higher in human services or related field preferred.
• Experience outreach to veterans and active-duty personnel is valued.
• Knowledge of VA health system is strongly preferred.

Responsibilities

• Assists therapists and Med Provider Team in meeting needs of veteran and active-duty military personnel.
• Provides home visits, outreach and follow up contacts with veterans and military personnel to assist in engaging them in their continuum of care needs.
• Develops CCBHC partnership, formal contracts and care coordination expectations with Department of Veteran’s Affairs medical centers, independent outpatient clinics, drop-in centers, and other facilities of the Department as defined in section 1801 of title 38, United States Code.
• Ensures there is integration or coordination between the care of substance use disorders and other mental health conditions for those veterans who experience both and for integration or coordination between care for behavioral health conditions and other components of health care for all veterans.
• Ensures every veteran seen for behavioral health services is assigned a Principal Behavioral Health Provider.

The Veterans Outreach Case Manager will provide direct support to armed forces, veterans and civilians with a focus on the former.
