Base Pay Range

$21.00/hr - $21.00/hr

Position Summary

The CCBHC Care Coordinator II provides case coordination, outreach, and advocacy services to individuals entering CCBHC. Responsibilities include initial screening, referral services, developing psychosocial evaluations, conducting health screenings, and creating treatment plans. The role supports a comprehensive set of services including crisis intervention, outpatient mental health and substance abuse treatment, and care coordination with community partners to improve outcomes and reduce healthcare costs.

Essential Duties and Responsibilities
  1. Analyze and interpret cases to determine necessary actions, exercising discretion and judgment.
  2. Work independently, evaluating options and making decisions without immediate supervision.
  3. Maintain an active caseload, providing referrals and linkage to services.
  4. Gather detailed consumer information to establish treatment plans.
  5. Implement and monitor treatment plans, ensuring services align with goals.
  6. Communicate with community partners and maintain positive relationships.
  7. Coordinate with community systems for linkage, referral, crisis management, and advocacy.
  8. Provide crisis management and linkage for interventions.
  9. Deliver individual and group psychosocial rehabilitative services, including skill training and mental health interventions.
  10. Maintain accurate records in compliance with standards and regulations.
  11. Uphold ethical standards regarding confidentiality and professional conduct.
  12. Demonstrate competent use of Electronic Medical Records (EMR).
  13. Participate in ongoing education and supervision meetings.
  14. Complete required training such as CANS/ANSA and DARMHA paperwork on time.
  15. Maintain a flexible schedule in collaboration with supervisors.
  16. Perform other duties as assigned.
Minimum Qualifications
  • Bachelor’s degree in relevant fields with at least 1 year of case management experience, or a non-specific bachelor’s degree with 4+ years of related experience.
  • Completion of case management training post-college.
Additional Requirements
  • Knowledge of community-based behavioral health care and case management.
  • Strong interpersonal and communication skills.
  • Willingness to use personal transportation, including for consumer transport.
  • Flexible scheduling.
  • Valid driver’s license and certifications such as CPI, CPR/First Aid.
Physical Demands and Work Environment

Must be able to communicate effectively, travel to consumer locations, and perform physical activities including lifting up to 40 pounds. Work involves variable conditions depending on treatment settings, with a 40+ hour work week, on-call duties, and periodic nights and weekends.
