Job description

The CCBHC Intake Specialist is responsible for ensuring efficient access to services for all clients, providing initial engagement and risk assessment. As an essential member of the CCBHC team, they provide a warm welcome to clients and families, coordinate a seamless admissions experience, and connect clients and families to necessary services, including those addressing social determinants of health.

Essential Functions:
  1. Serve as an integral member of the admissions team, collaborating with CCBHC psychiatric providers, clinicians, and administrators, as well as external programs, to ensure efficient and streamlined experiences for clients seeking services.
  2. Outreach, engage, and follow up with all newly referred clients and associated parties within required timeframes, continuously assessing for safety and priority, and triaging to support systems until a successful handoff to the permanent team.
  3. Administer initial screenings to assess risk, priority, and social determinants of health, connecting to clinical leads as necessary, and entering data into appropriate systems.
  4. Ensure accurate, complete, and timely documentation in agency data/tracking systems and Electronic Health Record (EHR), including client assessments and notes, in compliance with all regulations and standards.
  5. Establish and enhance collaborative relationships with external partners and service providers to promote optimal client care.
  6. Collect, analyze, and distribute admissions data regularly, focusing on efficiency, access to services, and client satisfaction.
  7. Perform other related duties as assigned.
Supervisory Responsibilities:

None.

Minimum Qualifications:
  1. Bachelor's degree in a Behavioral Health field, including social work, psychology, counseling, or public health.
  2. Proficiency in Teams and Microsoft Office.
  3. Excellent written and verbal communication skills.
Preferred Qualifications:
  1. Master's degree in a behavioral health field.
