The CCBHC Intake Specialist is responsible for ensuring efficient access to services for all clients, providing the initial engagement and risk assessment. As an essential member of the CCBHC team, they provide a warm welcome to our clients and families, coordinating a seamless admissions experience and connecting clients and families to all necessary services, including those addressing social determinants of health.

Essential Functions:

  • Serve as an integral member of the admissions team, collaborating with a team of CCBHC psych providers, clinicians, and administrators, as well as collaborative TCCNY & external programs to ensure efficient and streamlined experiences for clients seeking services.
  • Outreach, engage, and follow all newly referred clients and associated parties within required timeframes, continuously assessing for safety, priority and triaging to necessary support systems until providing a successful, warm hand off to their permanent team.
  • Administration of initial screenings to assess for risk and priority, as well as social determinants of health, connecting to clinical leads as necessary and entering the data into the appropriate systems.
  • Ensure the maintenance of accurate, complete and timely professional documentation in the agency data/tracking systems and Electronic Health Record (EHR), including client assessment and individual notes in compliance with all agency, city, state and federal regulations and standards of practice.
  • Work to establish and enhance collaborative relationships with external partners and service providers to promote optimal client care.
  • Collect, analyze and distribute admissions data on a regular basis with goals including, but not limited to efficiency, access to services, and client satisfaction
  • Other related duties as assigned.


Supervisory Responsibilities:

None.

Minimum Qualifications:

  • Bachelor's in Behavioral Health field including, but not limited to, social work, psychology, counseling, public health
  • Proficiency in Teams and Microsoft Office
  • Excellent written and verbal communication skills


Preferred Qualifications:

  • Master's degree in behavioral health field
  • Bilingual in Bengali, Spanish and/or Mandarin

  • Safe and successful performance, including meeting productivity standards.
  • Maintain an understanding of the principles, methods and processes needed to perform the job. Attends staff meetings, seminars and in-service training as assigned.
  • Demonstrates the ability to complete work in an acceptable manner by the time and/or date established with accuracy and minimal errors.
