Job description

CSOis looking for a Case Manager to join our PACT team in Florence.

Program for AssertiveCommunity Treatment (PACT) is an evidence-based intensive outpatient treatmentprogram designed to assist adults overcome the barriers to their recovery fromsevere and persistent behavioral health disorders.

This position willprovide intensive case management to adults 18 years of age and older needingsuch support to maintain their independence in the community, or who have beenhard to engage in the behavioral health system.

As a PACT Case Manager:

You Will:

  • Provide motivational counseling, prompting, andintensive support to consumers during psychiatric crises or periods oftransition; monitor and assist clients with personal hygiene and groomingtasks, grocery shopping and meal preparation, and household activities
  • Provide education and support with identifyingcommunity resources including appropriate mental health, addiction andmedical services, and accessing services of choice
  • Provide skills training and assistance in areas ofdaily living activities including basic living and interactive skills,providing "in vivo" skills training, side by side assistance,vocational and pre-vocational support to clients to include identifyinginterests and strengths and goals, and development of individualizedplacements. Assist in developing communication and conflictresolution
  • Provide assistance with developing and maintaining a"natural support network" of family and friends
  • Provide crisis and intervention treatment services asmembers of mini-treatment team
  • Assist, train and support clients in locating andmaintaining decent and affordable housing, negotiating leases, andobtaining furniture and other necessary fixtures; facilitate cooperativerelationships with landlords and housing authority as indicated
  • May provide recovery support services includingexploring treatment recovery options with consumers, developing a set ofgoals and objectives in conjunction with the consumer based on identifiedneeds, providing education and resources, and assisting consumers inaccessing treatment and community supports
  • Provide transportation for consumers as appropriate,following program guidelines
  • Maintain appropriate contacts with family members,others in the "natural helping network," outpatient therapists,and others as necessary to support consumers

Qualifications

  • Bachelor's degree in a behavioral health discipline orrelated field
  • Must be able to work flexible schedule which mayinclude evening and weekend work, as required by client needs
  • Valid driver's license and reliable transportation
