Qualifications

• Ability to use high-level abstract reasoning to assess patients and adjust plans.
• Basic computer skills navigating Windows-based programs.
• Must be fluent in English (read, verbal and written).

Responsibilities

• Demonstrates critical thinking appropriate to patient care.
• Collects and documents patient health data.
• Performs thorough assessments and develops evidence-based care plans.
• Implements care plans safely and documents actions.
• Promotes timely patient placement and discharge per hospital policy.
• Participates in peer interviewing and reviews.
• Administers medications and treatments as prescribed.
• Communicates professionally with patients, families, physicians and staff.
• Educates patients and families on health conditions and care plans.
• Maintains compliance with regulatory standards and policies.
• Participates in quality, safety and efficiency initiatives.
• Acts as patient advocate and ensures rights and needs are respected.
