Job summary

Northwestern Medicine is seeking a Registered Nurse with a BSN to join our patient-centered team in Illinois. You will perform comprehensive assessments, coordinate care, and ensure safe, evidence-based practices across the patient continuum.

You will collaborate with physicians, therapists, and support staff to deliver high-quality care, participate in discharge planning, and educate patients and families using best practices and portals for post-discharge support.

Qualifications

• Required BSN and Illinois RN license with active status.
• BLS certified; ability to collaborate with multidisciplinary teams.
• ADN may be considered if the candidate commits to pursuing a BSN.

Responsibilities

• Perform comprehensive patient assessments and document results.
• Communicate changes in patient condition promptly to the team.
• Develop and update the care plan with measurable goals.
• Collaborate with patient and family; plan discharge and referrals.
• Assess discharge needs and provide patient education and resources.
• Ensure accurate medication reconciliation and patient portal guidance.
• Participate in unit safety, quality initiatives, and huddles.

Education

BSN (Bachelor of Science in Nursing)

Illinois RN license

Basic Life Support (BLS)
