Qualifications

Responsibilities

• Assesses and analyzes the health status of patients in the assigned nursing unit.
• Performs ongoing assessments and documents according to policy and procedure.
• Initiates discharge planning with initial admission assessment.
• Plans patient care in collaboration with the patient, family, and health care team.
• Directs, implements and coordinates patient care activities to ensure quality care.
• Communicates the plan of care to other health care team members.
• Documents all patient care including assessments, plan of care, interventions and follow-up.
