Job summary

Community LIFE Tarentum, Pennsylvania is seeking a Casual Registered Nurse to coordinate nursing care and assist with transitions from home, traveling across Tarentum and Logans Ferry service areas as needed.

Under supervision of the Nurse Manager, you will document care, collaborate with clinic and home care staff, and provide prescribed treatments while upholding gerontological care standards. Schedule is casual with 16+ hours monthly and weekends/evenings availability preferred.

Qualifications

• Graduate of an accredited nursing program.
• Valid PA driver’s license.
• Minimum 1 year of experience with frail or elderly populations (home care, long-term care, or community health preferred).
• Automatic 403(b) Retirement Plan Contribution.
• Mileage Reimbursement at Federal Rate.
• Daily Pay Available.

Responsibilities

• Assists RN Case Manager in coordinating care to minimize unnecessary inpatient care and ensure smooth transitions home.
• Collaborates with clinic, day center, and home care staff to coordinate comprehensive participant care.
• Documents care provided and changes in a participant’s condition in a timely manner per standards and policies.
• Provide nursing care and prescribed treatments and demonstrate nursing care to participants and families.
• Assumes clinic nurse responsibilities in the absence of clinic nurses.

About this position

Position Overview: Under the supervision of the Nurse Manager, the Casual Registered Nurse (CRN) will assess the need for, and coordinate the delivery of, nursing care and home/personal care. The CRN must demonstrate the knowledge and skills necessary to perform assigned responsibilities according to the standards of practice in gerontological nursing and Community LIFE standards. The CRN must be able to travel to assignments as needed, covering multiple service areas as assigned.

Schedule: This is a casual position with a minimum requirement of 16 hours worked per month. We are looking for a candidate with flexibility, as this position will cover vacations, call offs, etc, so as much availability on days, evenings and weekends is preferred.

Location: Tarentum and Logans Ferry service areas

Key Responsibilities

• Assists RN Case Manager in the coordination of care to minimize or eliminate unnecessary inpatient care and accomplish smooth transitions to and from home.
• Interacts and works closely with clinic, day center, and home care staff to coordinate comprehensive participant care.
• Documents care provided and changes in a participant’s condition in a timely manner, according to accepted professional standards and the organization’s medical records policies.
• Provides nursing care and prescribed treatments and demonstrates nursing care to participants and caregivers/families.
