Job summary

Fairview Health Services is seeking an Occupational Therapist to support outpatient hand therapy across the Twin Cities metro with a flexible, casual schedule. The role emphasizes evaluation, treatment planning, and collaboration with a rehab team to help patients reach functional goals.

Preferred candidates are CHt or have NBCOT certification, with opportunities for mentorship and professional development within Fairview Rehabilitation.

Qualifications

• Minnesota OT license (or temporary license) is required.
• Basic Life Support (BLS) certification is required.
• Master's degree in Occupational Therapy and NBCOT certification are preferred.

Responsibilities

• Evaluate and re-evaluate patients to determine treatment, prognosis, and goals.
• Develop individualized plans of care in coordination with patients and families.
• Implement, modify, and document therapeutic interventions and education.
• Coordinate care with interdisciplinary team and discharge planning.
• Maintain timely documentation and comply with billing standards.

Job Overview

We are seeking an occupational therapist to serve our outpatient hand therapy patients with the rehab flexible workforce team!

Preferred candidate will be a Certified Hand Therapist (CHT). Where CHT is not completed, Fairview offers mentoring and development opportunities for professional growth. The therapist is responsible for providing safe and effective delivery of patient care including Examination, Evaluation, Diagnosis, Planning, Intervention and Establishing Outcomes.

• FTE: 0.01, casual position; authorized to work up to 40 hours per week.
• Schedule: As needed Monday - Friday, generally 8:00am - 4:30pm. Opportunities to pick up shifts as available and as able.
• Position may provide coverage for various locations across the Twin Cities metro area; including, Blaine, Burnsville, Edina, Eagan, Elk River, Maple Grove, Minneapolis, Princeton, Woodbury, and Wyoming.

Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Responsibilities

• The occupational therapist partners with the patient and family while performing the Evaluation/Re-evaluation
• Gathers pertinent data
• Performs examination
• Determines treatment diagnosis
• Develops an individualized plan of care through coordination with the patient and family
• Analyzes data to identify needs and potential risks
• Determines prognosis for recovery
• Establishes individualized goals with patient and/or caregiver
• Individualizes plan of care considering developmental level and cultural differences
• Implements and modifies plan of care based on reassessment and patient response
• Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
• Establishes appropriate discharge plan
• Partners with patients and families to provide skilled intervention

• Alters treatment/plan of care to reflect change in patient status and response to treatment
• Assesses patient and/or caregiver learning needs and provides appropriate education

Required Qualifications

• Occupational Therapy License in the State of Minnesota or temporary license in State of Minnesota
• Basic Life Support (American Heart Association or Red Cross)

Preferred Qualifications

• Master's degree in Occupational Therapy
• 1+ years of experience in Occupational Therapy
• CHT (Certified Hand Therapist)
• 1+ years hand therapy
• Certification with the National Board for Certification in Occupational Therapy
