Job summary

Momentum Community Connections is seeking a Disability Support Worker to deliver services that enhance living, social and recreational skills for people with a disability in Victoria. The role emphasizes inclusion, independence, decision making and personal choice aligned with individual needs.

The position focuses on promoting community involvement, safety and wellbeing, with a casual employment structure in the Western Suburbs of Melbourne.

Qualifications

• First Aid Certificate and CPR required or willing to obtain within six months.
• Cert III or IV in Individual Support or similar preferred (or willing to obtain one).
• Minimum 6 months relevant industry experience.
• Driver's License required.
• Reliable and comprehensively insured vehicle.
• Satisfactory Police Records Check.
• NDIS Worker Orientation Module Certificate or willingness to complete.
• Working with Children Check-employment required.

Responsibilities

• Provide personal support and assist with households as outlined in the participant's plan and encourage active participation.
• Assist participants with community participation to reach their goals and objectives.
• Provide support enabling participants to engage in community or daily activities of their choice; create a safe, enjoyable environment.
• Promote participant decision making, dignity and rights; show empathy and respect.
• Engage with participants and perform administrative duties.
