Job Summary
The Caseworker works in collaboration with the multidisciplinary team to coordinate mental health, behavioral, and social services for patients. The role facilitates communication between patients, families, healthcare professionals, community and state agencies, ensuring the implementation of care plans and overall patient advocacy. This position is a UNION role within the Care Management Department.
Responsibilities
- Participate as a member of a professional team providing evaluation and treatment for psychiatric patients, including psychological assessment, crisis intervention, case management, patient advocacy, care coordination, and community outreach.
- Provide input in treatment decisions; present specific treatment plans for approval.
- Collect and record information from patients and other appropriate sources to support treatment recommendations.
- Involve patients, family, and others in the delivery of care and treatment recommendations.
- Collaborate with patients to set achievable goals during the course of treatment.
- Arrange safe and expedient transfer of patients when required.
- Serve as a resource of information for division members, patients, referral sources, and agencies.
- Assist with provider schedule maintenance and appointment coordination.
- Support quality patient care and patient satisfaction, contributing to related organizational goals.
- Assist with insurance verification and authorization procedures.
- Ensure patients progress from acute episode of care through post‑discharge, maintaining quality while being efficient and cost effective.
- Act as a liaison between the hospital and community agencies, exchanging clinical and referral information.
- Identify high‑risk patients, assess psychosocial and financial needs, and provide counseling, care management, and referrals to appropriate resources.
- Resolve psychosocial patient care issues and develop and implement complex patient transition/discharge plans.
Qualifications
- Bachelor's degree in a related field of study (required).
- Master's degree in Social Work preferred.
- Union membership and awareness of union rates of pay based on education and experience.
Experience & Certification
Certification(s) and license(s) as applicable to the role. Experience in mental health case management and interdisciplinary team coordination is highly desirable.
