Job Summary

The Caseworker collaborates with the team to coordinate care for mental health, behavioral and social needs, facilitating communication between patients, families, healthcare professionals, community and state agencies. The Social Worker shares similar responsibilities. This is a UNION role within the Care Management Department, with non‑negotiable rates of pay based on education and years of experience.

Responsibilities
  • Perform psychological assessment, crisis intervention, case management, patient advocacy, care coordination and community outreach.
  • Obtain information from patients and other appropriate sources, record data in the medical record, and present treatment recommendations.
  • Involve patients and families in care planning, determine therapeutic goals, and arrange safe transfer and follow‑up treatment.
  • Assist with insurance verification and authorization procedures.
  • Act as liaison between hospital and community agencies for exchange of clinical and referral information.
  • Identify high‑risk patients from psychosocial or financial perspectives, provide counseling, care management and referrals to appropriate resources.
  • Develop and implement complex patient transition and discharge plans.
  • Support quality patient care, patient satisfaction, and related goals, ensuring efficient and cost‑effective service.
Qualifications
  • Education: Bachelor’s in a related field of study for Caseworker; Master’s (or Bachelor’s) in Social Work for Social Worker.
  • Skills: Patient Rights, Teamwork.
  • * Does not qualify for J‑1 waiver.
