Job summary

Lancaster County Government is hiring a Support Coordinator to assist families of children with developmental delays or disabilities. The role involves coordinating evaluations, IFSPs, and Transition Plans, and ensuring timely early intervention services.

You will gather assessment information, liaison with a multidisciplinary team, and enter data into LUIS, PELICAN, and EI Tracking while traveling to family locations as needed. A bachelor's degree in social sciences is required.

Qualifications

• Minimum qualifications include a relevant degree or approved combination of experience and coursework.
• Experience with family assessment and establishing IFSP/Transition planning is preferred.
• Ability to manage data entry and documentation accurately within required timeframes.

Responsibilities

• Gath er family assessment information and share with team before evaluation.
• Participate in initial/annual evaluations requiring travel to home or community locations.
• Develop, implement, and review IFSPs and Transition Plans.
• Coordinate data entry into LUIS, PELICAN, and EI Tracking databases.
• Ensure timely delivery of early intervention services and inform families of their rights.
• Maintain accurate paperwork and document family contacts every 90 days.
