Hartford HealthCare is seeking a case manager to join the dynamic PHP/IOP program at the Institute of Living in Hartford, Connecticut. The successful candidate will assist patients in navigating community resources and benefits, as well as providing crucial support for accessing transportation services.
A bachelor's degree in Human Services is required along with the ability to obtain a Public Service License. The position comes with medical, dental benefits, and generous paid time off.

The Institute of Living, one of the first mental health centers in the U.S., remains one of America’s leading not-for-profit centers for patient care, research and education in the fields of behavioral, psychiatric and addiction disorder. Through our dedicated, experienced and skilled clinicians, we provide patients and their families with the highest quality of personalized care. Programs are offered for children, adolescents and adults struggling with mental illness as well as offering services specializing in Anxiety, Eating Disorders, and Geriatric Services.
This position offers an opportunity to be part of a dynamic PHP/IOP program located at the Institute of Living. The case manager will be directly involved with assisting patients locate resources in the community and on campus, apply for additional benefits, access local transportation services to facilitate program adherence and complete paperwork for leave status.
