UAB Medicine in Birmingham, AL is seeking a skilled Care Coordination Specialist to coordinate, negotiate and obtain services across the care continuum for patients. The role emphasizes interventions to promote health, prevent disease, and address barriers to care, including discharge planning and community resource linking.
Qualifications include RN licensure (Alabama) or LMSW with healthcare experience; BLS; ACLS/PALS may be required; PSLF eligibility noted for eligible positions; sign-on
Pay range: $55,180 - $89,670 / year
Sign-on bonus: $7,500 sign-on bonus may be available
To coordinate, negotiate and obtain services and resources for the care of patients across the care continuum. To provide interventions designed to promote health, prevent disease, and address barriers of access to healthcare. To facilitate the discharge and or transitions planning process and identify the most appropriate community resources to assist patients and their caregivers. To evaluate medical necessity and cost effective methods of patient care while promoting value for the patient and the organization.
