Job summary

UAB Medicine in Birmingham, AL is seeking a skilled Care Coordination Specialist to coordinate, negotiate and obtain services across the care continuum for patients. The role emphasizes interventions to promote health, prevent disease, and address barriers to care, including discharge planning and community resource linking.

Qualifications include RN licensure (Alabama) or LMSW with healthcare experience; BLS; ACLS/PALS may be required; PSLF eligibility noted for eligible positions; sign-on

Qualifications

  • Bachelor's degree in Nursing and three (3) years of related experience required OR Associate's degree in Nursing and five (5) years of related experience required OR Master s Degree in Social Work required and 2 years experience in a healthcare care setting preferred.
  • Work experience may NOT substitute for education requirement.
  • Currently licensed as either a Registered Nurse (RN) by the Alabama Board of Nursing or eligible Nursing Licensing Compact or Licensed Master's Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
  • Nurses require certification in Basic Life Support (BLS) by the American Heart Association (AHA) or should be obtained with in 45 days of employment.
  • Nurses may be required to obtain Advanced Cardiac Life Support (ACLS) certificaiton and/or Pediatric Advanced Life Support (PALS) from the American Heart Association.
  • LMSWs will be required to complete the First FIve Healthstream within 45 days of employment.

Pay range: $55,180 - $89,670 / year

Sign-on bonus: $7,500 sign-on bonus may be available

Job Description
