Job summary

Road To Responsibility, Inc. is seeking a full-time Direct Care/Residential staff to support adults with developmental disabilities in a residential setting. You will perform case management duties, medication administration, and daily living support under the Residential Manager’s supervision.

The role requires a valid driver’s license, ability to document accurately, and commitment to safety. Training is provided, and a $2,500 sign-on bonus is available for new full-time hires.

Qualifications

  • Bachelor’s degree in human services or related field is preferred.
  • High School Diploma or equivalent with 2 years direct human services experience acceptable.

Responsibilities

  • Provide operational duties assigned by the Residential Manager.
  • Provide case management duties: attend meetings, write progress notes, develop supports and assessments, and track objectives.
  • Attend weekly staff meetings, document minutes, meet deadlines, and support program development and evaluation.
  • Maintain valid Driver’s License and provide transportation as required.
  • Oversee shift documentation to ensure accurate reporting.
  • Instruct, supervise and evaluate individuals in community living per support plans; record data.
  • May cover shifts off-site and assist with hospital stays when needed.
  • Administer medications per procedures.
  • Assist individuals with scheduling and attending social/leisure activities.
  • Maintain residence and yard in clean, orderly condition as outlined by the Program Manager.
  • Assist with personal care: diet, exercise, meals, bathing, dressing, toileting.
  • Assist with physical direct care including transfers as needed.
  • Participate in on-the-job training and coach new staff.
  • Adhere to communication lines, dress professionally, and project a positive attitude.
  • Write daily shift reports and other required documentation.
  • Adhere to agency safety standards and program routines.
  • Perform additional duties including snow/ice removal and debris cleanup.

RTR is offering a $2,500 Sign on Bonus for New Full Time Hires!
Act as part of a team responsible for the support, evaluation, and treatment of adults with developmental disabilities living in a residential setting. Perform all case management and operational duties assigned by the Direct Care/Residential Manager.

ESSENTIAL DUTIES AND RESPONSIBILITIES Include The Following
  • Provide operational duties assigned by the Residential Manager. This may include: ordering

medication, assisting individuals with banking, monitoring program money and petty cash, grocery shopping, assisting with training new staff.

  • Provide case management duties as needed. This may include: attending and participating in

meetings regarding individual(s), writing monthly progress notes, Individual Support Plans and assessments and objectives for annual reviews, assisting with the development of behavior plans or intervention strategies, collecting data, and completing all other paperwork as requested.

  • Attend weekly staff meetings, document meeting minutes, meet deadlines on all program related paperwork, and act as a team member in the development, implementation, and evaluation of programs and services.
  • Maintain valid Driver’s License and provide transportation as required by program.
  • Responsible for overseeing shift documentation provided by staff to ensure professional and accurate reporting.
  • Instruct, supervise and evaluate individuals in all areas of community living as outlined in each support plan. Record data on all instructional programs.

May include covering shifts off-site, any hospital stays (i.e., Boston).

  • Administer medications, as required, in accordance with medication administration procedures.
  • Assist and support individuals as needed with scheduling and attending social and leisure activities such as: family gatherings, religious celebrations, community events, and social gatherings.
  • Maintain residence and yard in clean, orderly condition as outlined by Program Manager. This may include: vacuuming, dusting, cleaning floors, bathrooms, laundry and keeping lawn clear of trash and debris. Also responsible for keeping steps, walks, paths and ramps clear.
  • Actively assist and support individuals as needed with personal care responsibilities. This may include: diet, exercise, meal preparation, bathing, shaving, dressing, toileting, etc.
  • Assist and support individuals as needed with physical direct care. This may include: physical or mechanical transfer of individuals from wheelchairs to a van, chair, or shower, etc.
  • Participate in on-the-job training and provide coaching to new co-workers as directed.
  • Adhere to all established lines of communication, dress in a professional manner, and represent a positive and proactive attitude with co-workers, families, and referral agencies.
  • Responsible for writing reports, including daily shift reports, and other required documentation

utilizing professional, and appropriate documentation skills.

  • Attend all required meetings, and adhere to all program routines and the schedules and needs of individuals under supervision.
  • Adhere to agency safety standards.
  • Perform additional duties, or substitute for others as required, including other programs.
  • Perform additional duties, or substitute for others as required. Additional duties may include but are not limited to clearing snow and ice from walkways, vehicles, parking lots, and roads using equipment like shovels and snow blowers while applying deicing agents to ensure safety
  • Adhere to all RTR policies and procedures.
  • Develop and provide all services in accordance with RTR’s philosophies including staff and Board beliefs
Qualifications
QUALIFICATIONS
  • Bachelor’s degree and one year experience in human services or High School Diploma or equivalent with a minimum of two years direct experience in human services
  • Basic knowledge of Microsoft Word and the internet.
  • Education related to a field such as: psychology, social work, special education or related human service work preferred, and at least one year residential experience.
  • Valid Driver’s License.
  • Successful completion and maintenance of all state and agency mandated trainings, including medication administration.
  • Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
  • Must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
Language Skills

Must be fluent in English and have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
