Job Responsibilities

Position Summary: Provide ongoing service coordination, treatment planning, advocacy, and monitoring to residents of Milwaukee, Waukesha, Racine, and Kenosha counties involved in various programs including OARS, CR, OCRP, and JBCR.

Essential Functions:

  • Coordinate, plan, and ensure follow-through with community treatment for program participants.
  • Develop supportive therapeutic relationships with each participant.
  • Incorporate evidence-based practices such as Motivational Interviewing (MI), Trauma-Informed Care (TIC), and Person-Centered Planning.
  • Utilize MI skills to enhance participant engagement and success.
  • Monitor treatment needs and compliance, including assessment, treatment planning, case management, symptom and medication management, crisis intervention, and team coordination.
  • Prepare and submit court documents for CR participants as required.
  • Coordinate team meetings with DHS/Department of Corrections and other treatment team members.
  • Maintain safety practices and assess risks in community settings.
  • Coordinate community support services and assist participants with daily activities such as housing, finances, and transportation.
  • Encourage development of natural support systems.
  • Maintain participant service documentation, including case notes, assessments, service plans, progress notes, crisis plans, and risk assessments.
  • Ensure compliance with program standards and documentation protocols.
  • Maximize participant financial contributions through insurance and community resources.
  • Communicate regularly with treatment providers, authorities, and community agencies.
  • Conduct home visits and transport participants as needed.
  • Develop MI skills through ongoing training and coaching.
  • Participate in coaching sessions, surveys, and complete required assessments.
  • Build positive relationships with stakeholders and funders.
Other Duties & Responsibilities
  • Collect medical records and lab results timely.
  • Participate in crisis line coverage and court testimonies.
  • Attend trainings, meetings, and staffings.
  • Support new staff onboarding and other duties as assigned.
  • Work may involve remote and community-based settings, including travel to various facilities and participant homes.
Qualifications

Knowledge, Skills, and Abilities:

  • Experience in mental health/substance use services, assessment, treatment planning, and medication management.
  • Understanding of legal and forensic psychiatric issues.
  • Resourcefulness, flexibility, and knowledge of entitlements like Medicaid, Medicare, SSI/SSDI, and Veterans benefits.
  • Strong communication, interpersonal, organizational, and computer skills.
  • Commitment to social justice.

Minimum Qualifications:

  • Bachelor’s Degree in a related human service field.
  • Experience in case management and serving individuals with mental illness or substance use disorders.
  • Valid driver’s license and insured vehicle.
  • Ability to pass background checks.
Additional Information

Work involves community and office settings, travel, and court appearances. Flexibility in work hours and participation in crisis coverage are required. Physical ability to drive, lift light loads, and assist participants is necessary.
