Job summary

Presents two or three paragraphs about Hartford HealthCare’s Institute of Living seeking a social services professional to join a multi-disciplinary team providing counseling, social histories, and referrals for patients and families in behavioral health. Bachelor’s in Human Services and licensure eligibility required; experience with diverse ages and community resources preferred; competitive benefits are offered in a central urban setting.

Qualifications

• Bachelor Degree in Human Services
• Ability to obtain Public Service License required
• Demonstrated competence including knowledge and behaviors to meet age-specific needs of the client population
• Experience working in related human service setting with similar age group

Responsibilities

• Provide routine social services as part of a multi-disciplinary team.
• Obtain social histories and provide counseling and guidance.
• Refer to agencies that respond to psycho-social needs of patients/clients and families.
• Identify or refer, or seek supervision on complex cases and report to Director.

Job Summary

Provides routine social services as a member of a multi-disciplinary team, including obtaining social histories, providing counseling and guidance, referrals to agencies that respond to the psycho-social needs of patients/clients and families. As appropriate identifies and refers, or seeks supervision on complex cases. Reports to Director of department or program, or designee.
