Qualifications

  • Background check through fingerprinting and enrollment in RAP Back program as per policy requirements.

Responsibilities

  • Provides information and assistance to clients seeking access to aging services and community resources.
  • Screens and counsels clients on program eligibility, including SHIP and health insurance options.
  • Responds to inquiries via phone, on-site visits, or walk-ins related to health, well-being, and safety.
  • Coordinates with Adult Protective Services, law enforcement, fire department, and other agencies as needed.
  • Provides community-based short-term case management and follow-up to meet identified goals.
  • Develops and maintains relationships with community partners providing services to older adults.
  • Attends community activities to educate the public about available services.
  • Maintains wait lists for services following established policies.

JOB SUMMARY

Provides County-wide information, assistance, and access to Aging Services and community resources for older adults. Assesses, plans, implements, coordinates, follows up, and evaluates options and services to meet clients' needs.

MINIMUM QUALIFICATIONS

Bachelor's degree from an accredited college or university in a Human Services field, Social Work, Sociology, Psychology, or other closely related field; OR an equivalent combination of related education and experience.

Due to the nature of this position, the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.

ESSENTIAL FUNCTIONS

  • Provides information and assistance to clients seeking access to Consults clients and families regarding service eligibility requirements.
  • Provides Senior Health Insurance Program (SHIP) screening and counseling including detailed information on Medicare, Medicaid, Prescription Drug coverage, advantage health plans, and other health insurance programs. Coordinates with SHIP volunteers to assist customers with all health insurance
  • Responds daily to telephone, on-site visits, and walk-in inquiries and concerns relating to health and well- being including emergency or potentially life-threatening
  • Coordinates appropriately to ensure customer safety including contacts with Adult Protective Services, Law Enforcement, Fire Department, Mobile Crisis Outreach Team, and other agencies as

  • Medicaid and Medicare rules and regulations
  • Assessment methodologies and techniques

  • Aging process, chronic illness, and related conditions
  • Geriatric home health services
  • Community resources, agencies, health care delivery systems, and rules and regulations
  • Interviewing methods and techniques
  • Computer and office software related to job specific duties

TEMPORARY POSITIONS:

An "At Will", FLSA non-exempt employee who is not eligible for County benefits and who must work an average of 29 hours or less per week or an average of 129 hours or less per month.
