Job summary

The Salvation Army in Lawrenceville, GA is seeking a Full Time Case Manager to supervise a caseload in a short-term life-management program. Responsibilities include eligibility interviews, intake planning, and coordinating services to help clients cope with their environment.

This role requires two years of college coursework in social work or related fields and two years’ experience in a social/public service setting, with a valid driver’s license.

Qualifications

  • Two years of college coursework in Social Work, Behavioral Science, or a related field.
  • Two (2) years’ experience working in a social or public service environment.
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Responsibilities

  • Interviews applicants to determine eligibility for program enrollment based on guidelines.
  • Develops intake and case plans for clients and completes necessary paperwork.
  • Prepares and maintains case records and logs; enters data into HMIS.
  • Meets regularly with clients to discuss progress and document outcomes.
  • Plans and facilitates life management classes to help clients cope with their environment.

This positionis responsible for:

Monitors assigned caseload of clients participating in a short-term life management program; establishes program plan/goals and evaluated client’s progress by conducting routine meetings with client and/or staff; serves as advocate for client to acquire services that will enable them to functionally cope with their environment.

Key Responsibilities:
  • Interviews applicants todetermineeligibility for program enrollment based on established guidelines and requirements; conduct needs assessment, obtain pertinentinformationandestablishprogram goals for eligible clients; refers applicants to other agencies if notappropriate forprogram.
  • Develops intake and case plans for client; instructs clients in completing necessary paperwork including goals and/or budget forms; completes orientation check-offform andobtains release of information forms whenappropriate; completes social history on new clients each week.
  • Prepares andmaintainscase records and logs on all assigned clients; ensures the accuracy and completeness of the same; enters pertinent information into the established Homeless Management Information System (HMIS).
  • Meets regularly with clients to discuss and evaluate their progress;preparesaccurateand up-to-date records documenting the same;receivesincident reports andenforcesdisciplinary actions for infractions of the house guidelines.
  • Plans, coordinates, and/orfacilitateslife management classes toassistclients in obtaining skills that will enable them to functionally cope with their environment;obtainsspeakers and workshop presenters as needed.
Physical Requirements and Working Conditions:

Ability to meet attendance requirements; Ability to read, write, and communicate the English language; Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) 5 -10% of work time; Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking. Work is performed in a normal office environment where there arelittle or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.

WHAT WE ARE LOOKING FOR IN YOUEducation and Experience:
  • Two years of college coursework form an accredited college or university in Social Work, Behavioral Science, or a related field.
  • Two (2) years’ experience working in a social or public service environment with experienceassistingthe public.
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
Licenses and Certifications:
  • Valid Driver’s License.
