JCCA in New York City seeks an Under-the-Line Case Planner to support foster youth in transitional placements. You will stabilize cases through expert case management, court advocacy, and hands-on support to ensure safety and well-being.

This field-based role requires flexibility, crisis response capabilities, and commitment to helping youth reach stability and permanency through coordinated services, home visits, and collaborative planning with partners.

Qualifications

  • Bachelor’s degree required; Master’s degree preferred.
  • Valid Driver’s License and willingness to travel.
  • Experience with vulnerable children, families, and diverse communities.
  • Proficiency in Microsoft Word and Excel.
  • Strong written, verbal, and organizational skills.
  • Calm under pressure and adaptable to changing environments.
  • Knowledge of child welfare systems and youth/family treatment approaches.
  • Flexibility to work evenings and weekends as needed.

Responsibilities

  • Serve as the primary liaison between JCCA, ACS, courts, and community partners.
  • Advocate in Family Court, represent the agency, and ensure court directives are implemented.
  • Lead and participate in Family Team Conferences, Initial Treatment Conferences, and Planning & Service/Red Flag meetings.
  • Conduct regular home visits, including to detention centers, hospitals, and locations outside NYC as needed.
  • Coordinate and supervise family and sibling visits.
  • Identify services and make referrals to support family and youth needs.
  • Maintain accurate, timely documentation in Connections and complete casework reports (FASPs, TPRs, court reports, etc.).
  • Respond to crises, implement AWOL protocols, and ensure safety measures are followed.
  • Collaborate closely with your team to develop individualized case management plans.
  • Support school placement and educational planning for youth.

Position Summary:
JCCA’s mission is to provide the highest quality child welfare and mental health services to New York’s most vulnerable children and families to ensure their safety, permanency, and well‑being, leading to a life of stability and promise.

The Under-the-Line Case Planner works with foster care youth in non-traditional or transitional placements—those who are AWOL, in detention, hospitalized, on trial discharge, or awaiting permanent placements. You’ll play a vital role in stabilizing these cases through expert case management, court advocacy, and hands‑on support.

This is a dynamic, field-based role that requires flexibility, strong crisis response skills, and a deep commitment to ensuring every child’s safety and future.
