  • Maintain case records and write psychosocial assessments as needed.

Position Summary:

JCCA’s mission is to provide the highest quality child welfare and mental health services to New York’s most vulnerable children and families to ensure their safety, permanency, and well-being, leading to a life of stability and promise.

The Under-the-Line Case Planner manages inactive or non-census foster care cases, including youth who are AWOL, in detention, on trial discharge, hospitalized, or in transitional statuses. The role focuses on ensuring safety, permanency, and well-being through case management, court engagement, and collaboration with ACS and other stakeholders. Flexibility, strong documentation, crisis response skills, and availability for evening and weekend work are required.

Key Responsibilities and Tasks:
  • Act as liaison with all stakeholders.
  • Represent team in Family Court Hearings and bring feedback from court to team for follow-up.
  • Organize and participate in Family Team Conferences and Initial Treatment Conferences.
  • Prioritize moving cases to stable permanency.
  • Identify cases that require and participate in Planning & Service and Red Flag meetings.
  • Assess service needs of family and child.
  • Conduct mandated home visits to family and child.
  • Conduct visits of youth who are in detention, hospital settings and at times out of county and/or state.
  • Provide referrals to family and youth for treatment and case management purposes.
  • Facilitate and supervise sibling and family visits as mandated.
  • Document all case activities in Connections.
  • Provide crisis intervention as needed.
  • Collaborate with team members to generate individualized case management plans.
  • Work with families to ensure appropriate school placement.
  • Implement AWOL protocol, including notifications, warrants, and Missing Person’s Reports.
  • Complete FASPs, Court reports, TPR summaries, and surrender applications.
  • Transfer incident reports into Connections progress notes.
  • Write Transfer Summaries as needed.
  • Complete Statistical Change Forms and CONX notes documenting movements of children.
  • Maintain case and accountability records per agency and government regulations.
  • Write psychosocial assessments as needed.
  • Other duties as assigned, including holding census cases when needed, evening/weekend hours, and travel outside the five boroughs.
Qualifications:
  • Flexible schedule required; basic hours 9 a.m.–5 p.m. with evening and weekend availability.
  • BSW or BA required; Master’s preferred.
  • Valid Driver’s License.
  • Strong organizational and follow-up skills.
  • Proficiency in Microsoft Word and Excel.
  • Ability to collaborate with culturally and professionally diverse teams.
  • Knowledge and skills in treatment with families and youth.
  • Excellent telephone, interpersonal, and writing skills.
  • Fast learner, adaptable to new tasks.
  • Bilingual (Spanish) preferred.

Hourly Rate: $27.4725 – $29.7115
