Utilize the integrated physical, behavioral health, and social determinants of health case management model to partner with members, providers, and caregivers. Key responsibilities include engaging members, conducting assessments, developing collaborative care plans to support self-management, promoting behavior change, ensuring care coordination, providing follow-up, and delivering clinical and quality outcomes, such as reducing emergency visits and readmissions and closing care gaps.
Provide initial screenings for case management services, assist members in accessing evidence-based care, collaborate with providers, and support case management activities. Tasks include inbound calls, digital communication, follow-up on care gaps and emergencies, addressing member questions, locating resources/referrals, and gathering medical records for reviews.
Premera offers extensive training, including integrated case management, motivational interviewing, and LEAN process improvement. Certified Case Managers (CCM) receive additional training to earn their credential.
