Job summary

NewPath is seeking a Case Manager to lead with purpose in our residential interventions program. You will conduct individual, family and group treatments, coordinate evaluations (DLA-20, ISP, PHQ9, Ohio Scales) and provide transportation as needed to reach goals.

Requirements include a high school diploma with 5 years of SED experience or a bachelor’s degree with 6 months SED experience; valid driver's license and insurance; willingness to work flexible hours and pass background checks.

Qualifications

• Bachelor's degree or equivalent in a relevant field preferred; experience with SED children.
• Ability to pass pre-employment background check and drug screen.
• Flexible availability including some evenings and weekends.

Responsibilities

• Conduct individual, family and group treatment interventions as indicated in the ISP.
• Monitor needs of clients and support systems; document and coordinate assessments.
• Provide transportation to reach goals and maintain professional relationships.
• Deliver culturally informed services; ensure ECR/EHR elements are accurate and up-to-date.
• Participate in supervision and adapt to new policies and procedures.

Education
