Job summary

Senior Solutions Vermont is seeking a dedicated Case Manager in the Windsor area to support older adults and individuals with disabilities. Candidates should possess a Bachelor’s in human services and have strong organizational and communication skills.

At Senior Solutions (Area Agency on Aging in southeastern Vermont), we are seeking a Case Manager to work with older adults and/or people with disabilities in Windsor and the surrounding areas using a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are required.

Qualifications

• Bachelor’s degree in human services and prior case management experience preferred.

• Passion for working with seniors or adults with disabilities, driven to learn new skills and quickly gain knowledge of benefits and services available to aging Vermonters.

Responsibilities

• Collaborating with other community partners to create a person-centered plan of care.
• Monitoring and reassessing program service plans as required.
• Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
• Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc.
• Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
