Starting Salary Range: $51,517 - $55,036 + Office Benefit Package (Parking/Remote Expenses): $130/Month subject to change

RADIAS Health is hiring a full-time Waivered Services Case Manager to join the Waivered Services Case Management team.
The Waivered Services Case Manager assists clients on a waiver to access needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source. The Case Manager supports clients who have been deemed at risk of placement in a Nursing Facility, Neurobehavioral Hospital or ICF/DD, and sets up and coordinates the services and medical equipment necessary in order for them to live in the least restrictive setting possible. The Case Manager also provides ongoing monitoring of services, and serves as an advocate for the client when assistance is needed or services are not being performed as required. Goal development and service planning utilizes Person‑Centered principles, and incorporates both what is important to and important for the client.
Hours: Monday - Friday 8 am - 4 pm or 9 am- 5 pm, 40 hours per week
